Please visit the City Ethics Commission website, via the link, to access the forms and instructions needed to complete the forms for the Statement of Economic Interest. http://ethics.lacity.org/SEIFiling/Filer/
Please note that you are only able to complete the form electronically and in one seating (saving is not allowable). Also, an originally signed document is needed, therefore, you need to print the form(s), sign the form(s), and submit to Valerie Steffens.
